Being the legal guardian and/or parent of the Player that I registered, I herby give my consent for his/her participation in the activities, as organized by the Heroes Sports Park.
I fully assume all risk, hazards, and medical expenses incidental to such participation, including transportation to and from activities.
I, the undersigned, understand that it is a requirement for my child to participate in the baseball and/or softball league’s annual fundraiser, to be determined by the Heroes Sports Park’s Board of Trustees.
I do hereby fully waive, release, absolve, indemnify, and agree to hold blameless Heroes Sports Park, sponsors, managers, coaches, Board of Directors, supervisors, umpires, and participants for any claim arising out of injury suffered during any activity or in transport to or from any activity.
I do hereby authorize the image and or name of player while participating in any said baseball or softball activity to be used by Heroes Sports Park for the league’s website. I understand that there will be no refund of registration or fundraiser money for any reason.
